If you’re facing something big like cancer, fertility care, heart surgery, hip or knee replacement, organ or tissue transplant, spine surgery, or weight loss surgery, Walmart’s Centers of Excellence (COE) program gives you access to specialists in some of the best facilities in the country, and many conditions are covered at no cost to you with most medical plans.1
The plan will only provide benefits for covered services relating to some of these conditions under the Centers of Excellence program. If you receive services outside of the Centers of Excellence program, the plan may not pay any benefit, unless an exception applies.

A medical record review for most cancer diagnoses is covered at 100% before deductible with most plans. If recommended, on-site evaluation and treatment are also covered. Localized skin cancers, basal cell carcinoma, squamous cell carcinoma, and localized melanoma are not included in the COE program.

Fertility treatments and services are covered at 75% after you meet your plan’s deductible with most plans. Covered services include but are not limited to in vitro fertilization (IVF) and intrauterine insemination (IUI). These services are subject to your out-of-pocket maximum. Fertility treatments and services, including IVF and IUI, are also subject to a $20,000 lifetime maximum benefit per person.
Eligible full-time associates also have access to benefits for surrogacy and adoption which do not require medical plan enrollment.
COE benefits shown above are available if you are enrolled in the Premier PPO, Saver HSA, or Contribution HRA plans. With the Personalized Wellbeing Copay plan you only pay the applicable copays for the services you receive. Your plan even covers travel and lodging for you and a caregiver. However, coverage for these services is only available under the COE program, which means your plan won’t cover care you receive anywhere else.

Certain heart surgeries are covered at 100% before deductible with most plans. If you are being considered for heart surgery, you and/or your doctor will be directed to Cleveland Clinic for a cardiac medical record review to make sure the treatment is appropriate. If Cleveland Clinic recommends heart surgery, you’ll be invited to have it done at Cleveland Clinic.
COE benefits shown above are available if you are enrolled in the Premier PPO, Saver HSA, or Contribution HRA plans. With the Personalized Wellbeing Copay plan you’ll have the same benefits, plus if you choose to have surgery somewhere else you’ll pay your plan’s regular copays. Hip and knee joint replacements through the COE program are covered at 100% before deductible with most plans. If you choose to receive care anywhere else, the level of coverage depends on your plan:
Premier PPO, Saver HSA, and Contribution HRA plans: If you receive hip or knee replacement services outside the COE program, benefits are reduced to 50% after out-of-network deductible. Coinsurance will not apply to your out-of-pocket maximum. In certain circumstances a network exception may apply.
Personalized Wellbeing Copay Plan: Coverage is only available under the COE program, which means your plan won’t cover care you receive anywhere else. In certain circumstances a network exception may apply.

Most organ and tissue transplants are covered at 100% before deductible with most plans. These include:
Liver
Kidney
Heart (including durable ventricular assist devices [VADs] and total artificial hearts)
Lung (including lung volume reduction surgery [LVRS])
Pancreas
Simultaneous kidney/pancreas transplants
Multiple organ transplants
Bone marrow/stem cell transplants (including CAR T-cell treatment).

Weight loss surgery performed through the COE program is covered at the regular benefit level of 75% of eligible in-network costs after you meet your plan’s annual deductible.
Coverage is only available under the Centers of Excellence program, which means your plan won’t cover care you receive anywhere else.
Weight loss surgery also has a 12-month waiting period from the date your medical coverage becomes effective before this benefit is available.
