Twin Health is available at no cost to you and your dependents aged 18+ if you need support for type 2 diabetes, pre-diabetes or weight management with a body mass index of 40 or greater and are enrolled in the Premier PPO Plan, Contribution HRA Plan, Saver HSA Plan, or the Personalized Wellbeing Copay Plan. You must also meet clinical eligibility requirements.
